model the completed fixed denture was constructed (Fig. 15-55). The prosthesis contained the two veneer crowns and their acrylic facings. Distal to the second bicuspid crown and fixed to it was a scalloped template with a few protruding vertical pin heads (Fig. 15-56). The superstructure was also prefabricated. For this case, it consisted of two molar crowns cast in one piece, which fit exactly over the template (Fig. 15-57).

On the third visit, the bicuspid veneer crowns with the extended template were fitted over the pre-pared teeth, and the occlusion was checked. All adjustments were completed prior to inserting the superstructure. With the superstructure in place, the occlusion was carefully checked and corrected. It was then removed, leaving the template in position.

Fig. 15-57. The two-unit molar superstructure fits exactly over the underlying template.

Fig. 15-58. The second premolar root is touching the floor of the maxillary sinus.

Fig. 15-59. An apicoectomy was performed on the root tip to make room between it and the antral floor. (From Linkow, L.I.: Atypical implantations for anatomically contraindicated situations, Dent. Concepts, Fall, 1967.)

Fig. 15-60. A, The framework was fitted into position and radiographed in order to determine where to drill the holes in the template to accommodate the pin implants. B, The framework was cemented into position over the two anterior bicuspid preparations after the holes were made through the template outside the mouth.
