524 Theories and techniques of oral implantology

A BRIEF REVIEW Guidelines for success

There are certain standard rules for using a blade implant. These are simple, yet essential to the long-term success of the implant intervention. They are basic, and the implantologist should rarely deviate from them. Briefly, the rules are these:

1. The fibromucosal tissues must be incised and reflected with clean, rapid surgery. The periosteum must be carefully separated from the underlying bone without mutilating or tearing it. Enough bone

should be exposed to clearly view the alveolar crest, labiolingual thickness, and topography.

2. The groove in which the implant will be inserted is made with a No. 700L fissure bur. A sharp bur with water is essential to avoid overheating the bone. The bur is channeled through the center of the alveolar crest deeply enough to bury the entire blade portion of the implant. Making the groove deeper is not deleterious as long as the buccolingual width of the blade is wider than that of the groove (Fig. 11-198). The groove should be thin bucco-

Fig. 11-201. A lateral oblique roentgenogram shows the alveolar crest covering the shoulders of the blade with at least 2 mm. of bone.
