Pterygoid extension

unilateral right endosteal blade

anterior

Anteriorly the site is limited by a wide open socket. A short span of bone exists between the open socket and an invaginated sinus (3, arrow). An open-socket bladevent is used to counterbalance the teeth, and a unilateral subperiosteal implant posteriorly (4-6) to avoid the fragile area. Thus a combination of implant types counterbalances natural teeth (8) to support a fixed prosthesis (10).
