A pterygoid extension implant need not be large or its use limited to cases where bone loss has been dramatically extensive. In this young person (1), the sinus had ballooned backward and downward into the tuberosity. Two teeth were prepared for inclusion in a splint from which had a cantilevered figure-eight female attachment for the pterygoid extension implant (2,3). The implant was inserted, the male attachment cemented into the figure-eight lock (4-6), and the splint fitted to it (7). With a minimal amount of preparation of natural teeth, a pleasing restoration without a bothersome saddle or clips was securely seated (8-10).
