Pterygoid extension with palatal struts

The patient found intramucosal inserts inadequate (1). Because her arch was very broad (2), the split-impression technique was mandatory, with pterygoid extension implant prototypes used (3,4). The impression included both prototypes (5). The fabricated implant (6) has an extensive palatal portion. Before insertion, copings were tried over the implant (7). The seated implant (8,9) fitted snugly, and the tissues healed without complication (10). The finished restoration with clip-on attachments was stable and far more satisfactory to the patient than her bulky, conventional restoration.
