The bulky denture was too heavy and awkward for stabilization with intrarnucosal inserts (1,2). The resorbed ridge was small (3) and irregular (4). Pterygoid extension implant prototypes (5,6) were used bilaterally in a split impression technique to get an accurate impression, as well as a bite (7). The prototypes were removed in the impression (8). Here the finished implant is reduced in its palatal portion to a flat strut that runs around the ridge from hamular notch to hamular notch (9). Labially three cross struts (10) stabilize the anterior portion of the implant. The implant is inserted (11), and the site closed (12) until it heals adequately for proper impressions for the final restoration. The patient is dismissed with a temporary palateless splint, which is used until the final restoration is finished and installed.
