A rather unusual form of early pterygoid extension implant was fashioned for this totally edentulous arch (1). The ridge was very uneven (2). An implant with an unusually long labial strut from cuspid to cuspid (3) was fashioned to fit the odd bone structure and rest on the anterior nasal spine (4,5). The posterior portions (6,7) involved the pterygoid extension concept. The healed tissues (8) display a bilaterally sturdy series of artificial abutments, to which the final restoration was cemented (9). Note that no struts crossed over the palate, a design concept in use today.
