Continual irritation (I) from the conventional denture made this patient's life miserable. Bladevents were inserted anteriorly and posteriorly on one side of the totally edentulous arch (2). Impressions were taken, and die pins representing the bladevents incorporated in the stone model (3). A pterygoid extension implant was fashioned, incorporating all the bladevents (4) and a unilateral subperiosteal portion. Note the bar arising from the hamular notch area, a feature which could interfere in some patients with the ascending ramus. The implant was inserted (5). When the tissues healed (6), impressions were taken for the final prosthesis—a denture (7-9) that locked onto the bar arising out of the soft tissues from the implant.
