In the old style pterygoid extension implant for a totally edentulous arch (1), the only bone adequate for sturdy bladevents in this case was in the canine pillar area. Here two open-socket bladevents were inserted (2). The implant was designed on a stone model (3) cast from an impression of the entire exposed bony ridge, including the hamular notch area. With hard cement inside the copings (4), the implant was inserted (5). The restoration was completed (6) and inserted (7). Although current procedures for such a situation have been greatly simplified, this old style implant still functions to the patient's great satisfaction.
