Case 17

A permanent full arch restoration for an edentulous mandible using vertical post implants

with a mesostructure

Sometimes it is necessary to fabricate a mesostructure over post type implants to more equitably distribute stress and to provide balance. The implants are placed into the bone so that their shafts protrude 4 mm. into the oral cavity. Interchangeable gold copings are placed over the protruding shafts.

A full mouth plaster index of the lower jaw and a wax interocclusal record of centric relation were made, and an alginate (irreversible hydrocolloid)

impression of the opposing jaw was taken. A scalloped template, which was to be tissue-bearing, was constructed and soldered to the copings. Protruding from the template between the copings were vertical posts that would help stabilize the final prosthesis. After this mesostructure template was tried in place over the implants, another bite was taken and the final prosthesis was fabricated, using the mesostructure as the framework. At the final visit, everything was cemented into position.

 

Case 18

A full arch restoration for the completely edentulous mandible using triplants

It is generally inadvisable to support a full arch with triplants. This type of implant must be set in
