The patient had a very unattractive maxillary situation (1). When the left cuspid was extracted, a bladevent was inserted between the midline and the socket (2). A double-posted, sinus-accommodating bladevent was inserted behind the socket (2). When the tissues adequately healed (3), impressions were taken for a right unilateral pterygoid extension implant to join with a three-quarter arch restoration. The restoration had a cantilevered female attachment (4) for anchoring the male attachment incorporated in the pterygoid extension implant (5). The pterygoid extension implant was inserted (6). After the tissues healed over the pterygoid extension implant, the remaining portion of the restoration was finished (7), and the occlusion carefully checked (8-10).
