Good bone under a sinus—enough for a standard or universal type bladevent—is rare, particularly in a totally edentulous case. Here the ridge is unusually wide (2), and it was possible to use bladevents around the entire arch. However wide the ridge appeared, however, the sinus dropped so low on each side that a universal type bladevent was moved back into the tuberosity on one side (3) and bilaterally a special sinus-circumventing design used (4). Anteriorly double-posted bladevents were used for heavy-duty support (5,6,7). When the tissues healed (8,9), an impression was taken for the final restoration (9,10).
